If you’ve ever been discharged from a Naples hospital with a new prescription, and then had to figure out how to fill it on your way home, NCH just changed that.
Naples Comprehensive Health has launched a “Meds-to-Beds” service in partnership with Publix Pharmacy, delivering newly prescribed discharge medications directly to patients at their hospital bedside before they leave. The service launched October 19 at NCH Baker Hospital and rolls out to NCH North Hospital on November 2.
How It Works
When a discharge prescription is ready, Publix Pharmacy representatives coordinate with NCH physicians, nurses, and care coordination teams to fill the prescription and deliver it to the patient’s bedside before discharge, not after.
A pharmacist is available at delivery to answer medication questions and provide counseling, reinforcing discharge instructions while the patient is still in the hospital. The pharmacy also works proactively to address insurance coverage, medication availability, and affordability concerns ahead of delivery.
The service is optional and carries no additional fee. Patients who use Meds-to-Beds are not required to switch to Publix Pharmacy for future prescriptions; they can continue with their existing pharmacy after discharge.
Why It Matters
Patients who leave the hospital without their prescriptions filled are significantly more likely to miss doses, experience complications, and be readmitted. Research has consistently shown that gaps in medication access following discharge, caused by transportation challenges, pharmacy hours, insurance issues, or simply forgetting, are a leading driver of preventable hospital readmissions.
“By bringing prescriptions and medication education directly to the bedside, we can address potential barriers before discharge and help patients feel more confident about continuing their recovery at home,” NCH said in announcing the program.
